Tablelands Community Support

Tablelands Community Support offers a range of services to assist older people, and younger people with a disability, to live independently in their homes. We offer services that provide you with the ‘Right Support, at the Right Time, in the Right Place’. We focus on your choices and your right to determine the supports that you access.

We want to help you to remain independent, and can offer a  range of services across the New England. We are funded by the government to provide Home Care Packages, which can assist you with Support Facilitation (planning and coordinating services), assistance in the home (personal care, domestic assistance, social support, respite and carer support.

Tablelands Community Support are also funded under the Commonweath Home Support Program, this program emphasises wellness and re-enablement. Support services offered under the program take into account your individual goals, preferences and choices. Carers are integral to ensuring the quality of life and independence their loved ones. The Commonwealth Home Support Program supports care relationships through providing respite care services for frail, older people, which allows their carers to take a break from their usual caring responsibilities.
